Damien Sports Medicine Partners with Pomona Valley Hospital Medical Center for Physicals

NOTE: If you are registered for the May 11 physicals, please pick up your documents at the attendance office.

Damien High School and the Physical Therapy Department at Pomona Valley Hospital Medical Center continue their annual partnership of providing physicals for current and future student-athletes.

Sports Medicine Center physicians, physical therapists and Rehab Services staff provide student-athletes with comprehensive, CIF required "Pre-Participation Examination" (PPE) sports physicals while raising money for Damien school through the SMC's "$$$ For Physicals" program. During the event, student-athletes make their way through height, weight, blood pressure, vision, ear-nose-throat, cardio and orthopedic check stations. Those who pass the exams receive a signed clearance from their team doctor good for immediate participation in summer practices, training camps and high school sports for the upcoming school year.

Appointment dates are Thursday, May 11 at 6:00 pm, or Monday, May 22 at 6:00 pm. Physicals are $25. You must register. You will need to bring the following: (1) completed parent release form; (2) completed Damien HS emergency card; (3) cash or check payable to PVHMC for $25; (4) glasses or contacts for vision test. Please wear comfortable, loose-fitting clothing. All Forms are available in the Damien Main Office (bldg. 600).
