Athletic Clearance

All student-athletes must complete the athletic clearance process outlined below before participating in any tryout, practice, or competition and no later than June 1, 2026. The athletic clearance process includes three required steps:
  1. Physical Examination (PPE)
  2. Online Athletic Clearance Submission
  3. Athletics Processing Fee Payment
All three steps must be completed before your student is cleared for any athletic participation.

Step 1: Physical Examination (PPE)
 
Per CIF Bylaw 503.G, all student-athletes must complete an annual preparticipation physical examination (PPE) verifying they are physically fit to participate in athletics.
 
Physical Exam Requirements:
  • Must be completed and signed by one of the following:
    • MD (Medical Doctor)
    • DO (Doctor of Osteopathic Medicine)
    • PA-C (Physician Assistant)
    • NP (Nurse Practitioner)
  • Physicals signed by a DC (Doctor of Chiropractic) or PT (Physical Therapist) will not be accepted
  • A hard copy must be submitted to the Athletic Training Office or Athletics Office
  • Physicals will not be accepted via email
 
Physicals can be completed by the following:
  • Your primary care physician
  • Pomona Valley Sports Medicine Center ($60)
  • Pomona Valley Urgent Care locations ($90):
    • La Verne - 2333 Foothill Blvd., La Veren, CA 91750
    • Claremont - 1601 Monte Vista Ave., Claremont, CA 91711
    • Chino Hills - 3110 Chino Ave., Chino Hills, CA 91709
  • Annual on-campus physical events held in May/June. Information will be provided when sign-ups are open.
 
Step 2: Online Athletic Clearance
All incoming and returning student-athletes must complete the online athletic clearance each school year prior to participation.
  • Visit www.homecampus.com 
  • Complete clearance process under Damien for the 2026–2027 school year.

3. Athletics Processing Fee
All student-athletes are required to submit the $200 athletics processing fee with the submission of the physical exam and online athletic clearance.This annual requirement funds administrative costs associated with the athletic clearance process and covers participation in multiple seasons of athletics in a year. This fee is separate from the participation fees for individual sports.
 
Important Details:
  • Must be submitted at the start of the student-athlete’s first sports season
  • Athletic clearances are not processed until this fee is received
  • Any unpaid fees will be charged to FACTS and will include a $50 late fee
